Transforming America by redirecting wasted health care dollars

ScienceDaily (Oct. 29, 2012) ? The respected national Institute of Medicine estimates that $750 billion is lost each year to wasteful or excessive health care spending. This sum includes excess administrative costs, inflated prices, unnecessary services and fraud -- dollars that add no value to health and well-being.

If those wasteful costs could be corralled without sacrificing health care quality, how might that money be better spent?

In a study published in the current online edition of the American Journal of Preventive Medicine, Frederick J. Zimmerman, professor and chair of the department of health policy and Management at the UCLA Fielding School of Public Health, and colleagues outline some of the myriad ways that $750 billion could benefit Americans.

"If cut from current health care expenditures, these funds could provide businesses and households with a huge windfall, with enough money left over to fund deficit reduction on the order of the most ambitious plans in Washington," Zimmerman said. "The money could also cover needed investments in transportation infrastructure, early childhood education, human capital programs, rural development, job retraining programs and much more. And it could transform America with little to no reduction in the quality of, or access to, health care actually provided."

Zimmerman noted that while different observers would likely have different priorities regarding the alternative uses toward which the wasted expenditures could be directed, all would agree that the alternatives proposed in this study have inherent social value.

"When the fastest-growing part of the economy is also the least efficient, the economy as a whole loses its ability over time to support our current living standards," said Jonathan Fielding, a UCLA professor of health policy and management and director of the Los Angeles County Department of Public Health, who is a co-author of the study. "The U.S. has become irrationally attached to its inefficient health care system. Recognizing the opportunity costs of this attachment is the first step in repairing the system."

In the study, the research group, which also included Dr. Steven Teutsch, chief science officer of the Los Angeles County Department of Public Health, and first author Jeffrey C. McCullough, a graduate student at the UCLA Fielding School, presented one scenario of how that money could be used.

For one, the authors propose that more than $410 billion per year -- or 55 percent of the savings -- could be returned to the private sector for individuals and companies to use as they please; another $202 billion (27percent) could go toward deficit reduction, yielding a greater reduction than the congressional "super committee" sought and failed to achieve. An additional $104 billion (14 percent) could support additional investments in human capital and physical infrastructure.

"For example," Zimmerman said, "the Head Start program could be doubled in size, universal preschool could be provided, average class size could be reduced from 22-25 to 13-17 students. And trained nurses could conduct regular home visits for high-risk pregnancies."

Two percent of the savings, amounting to $18 billion, could promote urban and rural quality of life by improving the built environment surrounding schools, expanding and modernizing public libraries, improving wastewater treatment and providing rural development grants to every small town in the nation. Job-training opportunities would be affordable for nearly 50,000 unemployed persons. And under the research group's scenario, the remaining 2 percent of the savings would be devoted to fully funding an extensive wish list of transportation projects to alleviate road congestion and promote mass transit alternatives.

Freeing up this money would be no easy task, Fielding warned. These excess expenditures will be difficult to reduce because the costs are spread across many groups, and the financial beneficiaries are coordinated, clear-minded and powerful, he said. Overcoming this resistance will require concerted collective action on the part of many economic sectors, governmental agencies and other organizations that are not used to seeing themselves as sharing interests with the others.

But whatever one's values and preferences, said Zimmerman, "eliminating excess medical care costs provides a monumental opportunity to reallocate those resources to strengthen our international competitiveness, enhance our well-being and build a healthier nation."

The result of redirecting some $750 billion per year, he said, could be transformative for Americans, and the potential uses for these funds are panoramic in both scope and possibility.

"This will not be an easy fight," Zimmerman said. "But we believe reconceptualizing our excess health care spending by looking at its opportunity cost to society is an important first step."

David Byrne and St. Vincent had to postpone their appearance on The Late Show with David Letterman tonight in New York because one of their backup musicians was stuck in Mexico City, a monitor engineer was stranded in San Francisco and a horn player flew from Portland to Minneapolis to Washington, D.C., before finally realizing he couldn't even get into the city via car. "A lot of the alternative plans that got made were then themselves canceled," says David T. Viecelli, one of the duo's managers, adding they'll still appear Thursday on The Colbert Report and hope to reschedule Letterman. "It was a constantly rolling readjustment period ? 'we have to do this more convoluted thing.'"

In addition to drowning subways, knocking down trees, exploding transformers and generally disrupting life in New York and the East Coast, Hurricane Sandy has scrambled the entertainment business, particularly music. Broadway shows and performances at Carnegie Hall, Radio City Music Hall and Lincoln Center were canceled on Monday and Tuesday; late-night hosts Letterman and Jimmy Fallon performed their regular shows yesterday with no live audiences, while Jimmy Kimmel nixed a live taping of his show in Brooklyn;?Journey canceled their show tonight at Brooklyn's Barclays Center; and Louis C.K., Mos Def, Ghostface Killah, and Grimes all canceled Monday night gigs.

Hurricane Sandy Disrupts East Coast Concerts

Other acts are cancelling shows outside of the New York metropolitian area. Bruce Springsteen and the E Street Band postponed their Rochester, New York concert from Tuesday to Wednesday; Trey Anastasio moved his Sunday night show in Port Chester to November 5th; Cat Power's Sunday gig in Philadelphia was pushed to January 2nd; and indie-rockers the xx and?electronic dance music star Pretty Lights scrapped tour dates throughout the East Coast.?

Perry Greenfield, who manages singer-songwriter Rufus Wainwright, spent the last two days with nowhere to go because his Red Light Management office is on Sandy-ravaged Wall Street. Wainwright's appearance, with fun. and the National for the Freedom to Love Now! marriage-equality benefit at the Beacon Theatre, was canceled tonight ? and Greenfield had to deal with the logistics from his Manhattan apartment with no Internet or cable. "It was tough yesterday," he says by phone. "We had a few stranded folks."

In the short term, the canceled concerts will merely be a financial hit for artists, venues and promoters. When New Yorkers and other residents in Sandy-affected states assess the damage, some predict longer-term repercussions. Sarah Kesselman, director of events and talent buyer for Santos Party House in Manhattan, had to postpone a sold-out Shiny Toy Guns show tonight and may not reopen until Thursday ? tough to overcome during Halloween week. "It's not that there is damage to the club, but we don't have power, so it's just a matter of canceling all the concerts," she says. "It is affecting us financially." A top promoter told Billboard he expected "an impact on our budgets and earnings."

Smaller clubs in especially rough weather zones may have longer-term problems, too. Owners of New York fixtures such as legendary jazz club Village Vanguard, Arlene's Grocery and Terra Blues couldn't be reached Monday, but Greenfield fears the worst. "It's in a basement in the West Village," he says. "Who knows what's going on." John Scher, a longtime New York City promoter who had no power in his suburban home, is even more apocalyptic. "New York's a disaster. Manhattan's closed down," says Scher, who canceled three shows in the city this week, including Rickie Lee Jones Thursday at the Concert Hall. "There's no power in Manhattan from 34th Street south ? that pretty much makes about 90 percent of the clubs in the rock space, and the subways are completely closed and flooded. Losing a week of shows probably will hurt a lot."?

Higher menu prices. Fewer work hours and jobs. Zero expansion and possible closings. This, writes a local franchiser, will be the potential impact in the restaurant industry of health mandates under the Affordable Care Act. But the co-founder of a workers? advocacy group suggests that businesses can profit by taking care of personnel.

Repeal Obamacare

More than three years into our jobless ?recovery,? 12.1 million Americans are still out of work. Nearly 23 million have stopped looking or can?t find full-time work.

The labor participation rate is 63.6 percent, the same level we saw in 1981. Employers are only adding slightly more jobs per month needed to keep pace with normal labor-force expansion.

So why did the unemployment rate go down below 8 percent last month? In large part, due to an increase in part-time work. While the drop in unemployment may seem completely positive, there is an underlying problem for small business owners due to Obamacare.

As employers begin to make their growth plans for the next year, the true costs of Obamacare have started to sink in. They threaten much of the growth forecast in businesses like mine, a franchise of pizza, burger and chicken restaurants that include Atlanta-based Popeyes Louisiana Kitchen. I want to grow my business and add more locations. Obamacare penalizes me every time I add a full-time worker to my franchise.

The employer mandate forces employers to provide coverage or pay a penalty once they reach the 50 full-time-employee threshold. Employers face a decision of closing a business, eliminating jobs, or shifting workers from full- to part-time to avoid the law?s penalties. We are likely going to choose the latter option.

In the franchise industry, there are thousands of multi-unit franchises like me who are put at a competitive disadvantage by the law?s employer mandate. This threatens growth in our industry. In cases where a franchisee owns and operates multiple locations, the law treats these firms as one company for tax and health-care purposes.

Suppose a multi-unit franchisee owns four establishments with 15 full-time employees each. Under the law, this multi-unit franchisee will be treated as a single firm with 60 full-time employees. The employer will be required by law to provide health care benefits for all employees or pay a fine of $2,000 per full-time employee per year. If these four establishments were owned and operated separately, they would be exempt from the health care requirement.

If these four separately owned businesses choose to offer health insurance, they would in many cases be entitled to a tax credit, making this an even further disadvantage for multi-unit ownership.

Businesses cannot operate at a loss for an extended time. That is the choice this law may force companies to face. Another often-overlooked factor is that many businesses will have an added incentive to become more automated and employ fewer workers. Utilizing automated check-out counters and purchasing new machinery are options we must consider to reduce the number of employees and remain profitable under these mandates.

Business owners like me who want to continue to expand would feel more confident about expansion and hiring plans without Obamacare. The only way to change this is by repealing Obamacare in its entirety and starting over with market-based solutions.

Health care can help stabilize workforce

There?s been a lot of talk about the recent announcement by Darden, the world?s largest full-service restaurant company and owner of Olive Garden and Red Lobster, that it would reduce workers? hours to avoid having to provide health coverage when the Affordable Care Act goes into effect in 2014.

None of the coverage mentioned the impact on one key stakeholder group: consumers.

Last fall, a server at a North Carolina Olive Garden was forced to work with Hepatitis A because the company neither offers paid sick days nor any kind of health care. Three thousand people had to get tested for Hepatitis A and many ended up winning a consumer class-action lawsuit against the company.

The Affordable Care Act (ACA) is a great step to ensure that people who touch our food are not sick, making sure that we don?t get sick as well. Darden claims the ACA is forcing the company to reduce its employees? hours to stay afloat.

Moreover, the National Restaurant Association would have you believe the industry is going to collapse if it actually provides workers with genuine health-care access. There are plenty of great employers in America already providing benefits.

For the last three decades, Zingerman?s Community of Businesses in Michigan has been providing up to 80 percent of workers? health care premiums on a comprehensive health care plan. Zingerman?s started as a two-employee deli in 1982. It has grown to an award-winning company with nine businesses that employ 575 workers and realize more than $40 million per year in revenue.

How? Zingerman?s was committed to working benefits into its business model from the start and heard workers? input in choosing the plan.

Says Zingerman?s founder Paul Saginaw, ?What do you get from it? You get a stable workforce. You get a workforce that can stay healthy. You get someone who feels good about the company and is out there trying to help the company be successful. The benefits are enormous. So now you figure out how to make it work.?

Everyone agrees health care costs are high. Saginaw believes a government-funded plan would be best. Without that, Saginaw needs and wants his employees to be healthy, for the sake of his workers and customers, and for his own bottom line. His workers say their customer service is better because they feel secure.

In fact, restaurants in the whole city of San Francisco have had to contribute toward a health care fund for their workers since 2008. Business is booming.

We should support restaurants that do and let others know they shouldn?t cut corners because we put our health in their hands every time we eat out.

Ten years of assisted suicide at Dignitas ... - NRL News Today

Editor?s note.? Dr. Saunders is a former general surgeon and CEO of Christian Medical Fellowship, a UK-based organization with 4,500 UK doctors and 1,000 medical students as members.

Now that the Tony Nicklinson case is over and the next assisted suicide bills (from Falconer in the House of Lords and Macdonald in Scotland) are not to be debated until next year one could be forgiven for thinking that the relentless media pressure for the legalisation of euthanasia might relent for a few weeks.

But no ? first we have the pronouncements of junior health ministers Norman Lamb and Anna Soubry giving their support for the legalisation of assisted suicide and now the BBC, in its role as cheerleader for assisted suicide, is making an international news story about the fact that it is ten years since the first Briton went to the Dignitas suicide facility in Zurich to kill himself.

This is actually a non-story. The on-going sad procession of desperate Britons on the Swiss suicide trail is in reality a tiny trickle but every new case is seized upon by sections of the British media, and especially the BBC, in an attempt to create the impression that there is a huge ?unmet need? and as a further opportunity to say again that we must have the debate that we have actually not stopped having constantly for the last six years.

In fact, given that the BBC repeatedly breaches international suicide prevention guidelines in its coverage, it is not beyond the realms of possibility that a significant number of those travelling abroad have been encouraged to do so by seeing others doing it on television with full media and celebrity support.

The carefully orchestrated and well-funded celebrity driven campaign run by Dignity in Dying, the former Voluntary Euthanasia Society, is skilled at getting hugely disproportionate coverage for every new case it can find and the BBC, in particular, is only too willing to provide them with an international media platform.

Today?s story is in the news because DID obtained some new figures from Dignitas and sent out a press release to sympathetic media outlets including the BBC. The BBC have also given prominence to an emotive hard case in a prime time slot in Radio four whilst relegating a more serious examination of the issues to a time when far fewer people were listening.

The reality is that between 2002 and 2011 a total of 182 Britons ? on average 18 per year ? have killed themselves at Dignitas. Numbers per year since 2006 have been relatively constant between 20 and 30 per year.

These numbers are a tiny fraction of the 550,000 natural deaths that occur in Britain each year and a very small trickle compared with the 650 and 13,000 who, on the basis of the 2005 Lords Select Committee report, it was estimated would die in Britain annually under an Oregon or Dutch-type law, respectively.

The 750% increase in assisted suicides amongst Swiss nationals since 1998, along with the disturbing 18% annual increase in euthanasia in the Netherlands over the last year will sound strong alarms to legislators in Britain that we should not be contemplating going down this route.

Reports from the Netherlands of psychiatric and dementia patients being euthanized and mobile euthanasia clinics along with Belgian accounts of organs being harvested from euthanasia patients and 32% of all euthanasia deaths being ?without consent? understandably fuel this concern.

Public opinion polls supporting a change in the law can be easily manipulated when high media profile (and often celebrity-driven) ?hard cases? are used to elicit emotional reflex responses without consideration of the strong arguments against legalisation.

But when these arguments are heard decision-makers have consistently voted against. There have been over 120 attempts to legalise assisted suicide through US state parliaments all of which have failed in the last fifteen years.

British parliaments have also rejected any loosening of the law here three times over the last five years ? in 2006, 2009 and 2010 ? on the basis that any change would place pressure on vulnerable people (those who are elderly, disabled, sick or depressed) to end their lives for fear of being a financial or emotional burden on others.

More than seven out of ten MPs refuse to back calls to legalise assisted suicide as shown in a recent ComRes poll.

The vast majority of UK doctors remain opposed to legalisation along with the British Medical Association, the Royal College of Physicians, the Association for Palliative Medicine and the British Geriatric Society.

Similarly all major disability rights groups in Britain have resisted any change in the law believing it will lead to increased prejudice towards them and increased pressure ?to seek help to die?.

The first duty of Parliament is to protect its citizens. Elder abuse and neglect by families, carers and institutions is real and dangerous and a law allowing the active ending of life could be so easily exploited and abused. This is why strong laws are necessary.

The British Suicide Act is thereby shown to remain fit for purpose. Through its blanket prohibition on all assistance with suicide, it continues to provide a strong deterrent to the exploitation and abuse of vulnerable people whilst giving both prosecutors and judges? discretion in hard cases. It strikes the right balance, is clear and fair and does not need changing.

Ten years of Dignitas and ongoing media pressure should actually change nothing.

153,000 Premium Domain Names Owned By Ondova Hits The ...

The U. S. Bankruptcy Court has just issued a Court Ordered for the sale of two portfolios of domain names, one containing more than 150,000 domain names and separate portfolio of more than 3,000 domain names (the ?Portfolios?), plus related monetization and registrar contracts.

SAN ANTONIO (AP) ? Motorists, start your engines. It's opening day for the last leg of a Texas toll road with the nation's fastest speed limit.

The final 41-mile stretch of Texas Highway 130 will open Wednesday after three years of construction. The speed limit will be 85 mph.

The toll road is intended to help alleviate the increasingly crowded Interstate 35 corridor between Austin and San Antonio, which are two of the state's largest metropolitan areas.

The new stretch will be from Mustang Ridge, about 10 miles south of Austin's international airport, to Interstate 10 just east of Seguin. A 50-mile stretch bypassing Austin is already open.

Tolls for personal vehicles displaying an electronic toll tag will be about 15 cents per mile. But no tolls will be collected until Nov. 11.

Healing Those Stubborn Emotional WoundsMary was experiencing low self-esteem and worthlessness.

She?d say her eyes were ?broken faucets.? She?d cry often, and would easily get irritated and explode at her children and husband. She had gained weight in the past year. She snacked all day and would finish a bag of chips in minutes without even noticing. She had difficulty concentrating, felt muscle tension, and above all, she was feeling like the ?worst mother in the world.? One day she reported she just wanted to ?escape her world.?

She was not suicidal but just wanted a break. She didn?t see the light at the end of the tunnel any time soon. Sometimes she would say things like ?I feel my heart being crushed. I am a bad person. I am exhausted and ready to quit. Things haven?t gone my way.?

Why was Mary so depressed?

She had a supportive husband and three children. Her husband had a good job and she had not worked outside of the home since she got married. Everyone in her family was in good health ? except her. Her adolescent son was making choices she didn?t like. His latest choices were against the family?s religious values and beliefs. It pained her to see her ?baby? go against what she taught him for seventeen years! She felt like a failure.

Mary was attributing her son?s misbehavior to herself. Intellectually, she agreed her son had a choice. After all, she had taught him about freedom and power to choose. But her guilt kept nagging within her: ?It didn?t do me any good to stay home. Maybe if I had worked outside of the home he would not be so spoiled. I should have been stricter. I should have spent more time with him. I should have home-schooled him.?

There were a lot of regrets, tears, and grief. Her handsome, well-groomed, intelligent, and healthy-looking son was becoming a memory. Things were turning out differently than expected. Her pain was intolerable and she tried to hide it. Arguments, yelling, and silence were only increasing the emotional gap between mother and son.

She was focusing on the half-empty glass.

She rehearsed in her mind all that ?she should have done, should not have done, and could?ve done.? Mary was focusing on the half-empty glass. She could not forgive herself. Her thoughts were getting her deeper and deeper into a black hole. At night she would go over things she had said or may have done in the past that ?could?ve caused? her son to take a different path than what she had anticipated for him.

She found herself in my office at her husband?s request. She actually didn?t believe she needed help. She believed she was a strong woman and could handle any trial. ?After all, isn?t that what life is about? We need to endure our trials, don?t we?? she?d say. I agreed with her, but reminded her enduring to the end doesn?t mean we have to suffer in guilt and pain the rest of our lives. She was experiencing many errors in her thinking.

Mary?s story represents stories from other women I have seen and counseled in my office. Many of them are experiencing emotional pain, difficulty sleeping, concentration problems, anger, regret, guilt, shame, anxiety, fear, loneliness, discouragement, and more.

The good news is that there is hope, People who suffer from depression and anxiety disorders can learn the skills to overcome their challenges.

Mary learned what was going on in her body and determined she needed to see her physician for medication. She made a plan to improve her physical condition by eating healthier and exercising. She learned relaxation skills such as progressive muscle relaxation techniques, visual imagery, and simple mindfulness exercises. Her sleeping habits also needed some readjustment.

She had forgotten to take care of herself and find joy. She took small steps to attain goals to develop new hobbies. She resumed activities she had abandoned. She had previously believed that she should focus all her time and attention to help her son figure things out. She realized her son needed to do most of the work. She finally recognized how her thinking errors and self-defeating beliefs had affected her emotions negatively. She identified the source of her negative core beliefs. She was able to change them by working diligently outside the weekly sessions. She learned a series of writing exercises and became an avid writer; that contributed to her healing also.

If you can relate to Mary?s feelings, remember there are answers ? there always are. Talk to a loved one you can trust. He or she can help you find the right therapist for you. It is possible to have joy despite the trials and challenges of life.

Kelsay said he thinks it may be on players like him to personally get in the faces of teammates who aren?t giving their all.

?We might have to grab guys by the shirt collar and tell them to get to moving,? Kelsay said. ?It?s nothing personal, but we?re relying on you. Myself included. ?I?d take Mario Williams at full health and full speed probably over anybody else in the league,? Kelsay said. ?At the same time I?d take Sean Ferguson, who we cut, over a guy that?s playing 50 percent, and I think our coaches would too. The playmakers have to make plays and the way you do that is giving all out effort every play.?

Ultimately, Kelsay says, there?s just no way he can tolerate being part of a defense that plays the way Buffalo?s defense is playing.

?It?s inexcusable,? Kelsay said. ?It?s a blessing to be in this league and to go out there and lay an egg there are no excuses for it.?

Kelsay is saying what needs to be said. The question is whether his teammates are listening.

Virtues of the Virtual Autopsy

Once a common medical procedure, the standard autopsy is passing out of use. In the 1970s bodies underwent postmortem examination in nearly 20 percent of deaths in the U.S. By 2007 the rate had fallen to 8.5 percent of all deaths and to only 4.3 percent of deaths caused by disease.

The reasons for the decline are well documented. Autopsies reveal medical mistakes, making doctors and hospitals uncomfortable. Medicare and private insurance do not reimburse providers for the procedures, so families must pay in full. And in the increasingly diverse U.S., members of some religions, such as Orthodox Judaism or Islam, object to dissecting a body after death.

Yet autopsy is a time-honored and reliable tool for confirming, or questioning, the actions of both medicine and law enforcement, so pathologists have looked for a viable alternative. Inspired by rapid technological improvements, researchers in several countries have been exploring the possibility that medical imaging?in particular, MRI and CT scans?might substitute a ?virtual autopsy? for the more traditional variety.

?The findings so far are mixed,? says Elizabeth Burton, a visiting associate professor of pathology at Johns Hopkins University. Virtual autopsy, she says, ?is better for examining trauma, for wartime injuries, for structural defects. But when you start getting into tumors, infections and chronic conditions, it's not as good, and I doubt it will ever be better.?

After about a decade of research, proponents concede that various difficulties?including high cost, competition for access to imaging machines and some inherent limitations of the technology?will likely prevent virtual autopsies from fully replacing the hands-on version. Nevertheless, the new techniques are answering cause-of-death questions that have frustrated traditional autopsies and are sidestepping religious objections. By enhancing medical education and suggesting improvements in emergency care, virtual postmortem examination is helping the living, too.

Forensic Frontier

Postmortem imaging began as a laboratory technique in legal investigations. Dissection usually destroys tissues, but a research group in Bern, Switzerland, recognized that advances in imaging technology would let them look deep into tissues while preserving evidence. In the early 2000s they combined MRI and CT scanning with computer-aided 3-D reconstruction to prove causes of death for difficult cases, which included drownings, flaming car crashes, and severe injuries to the skull and face.

Their process, which they dubbed ?virtopsy,? ignited interest in applying postmortem imaging to other forms of traumatic injury. Since 2004 the U.S. military has performed x-rays and CT scans on the bodies of every service member killed where the armed forces have exclusive jurisdiction?that is, not just on battlefields abroad but on U.S. bases as well. Imaging ?is an adjunct to the traditional external and internal postmortem exam,? says Edward Mazuchowski, chief deputy medical examiner in the Armed Forces Medical Examiner System. ?It allows us to identify any foreign bodies present, such as projectiles. X-rays give you the edge detail of radio-opaque or metallic objects, so you can sort out what the object might be, and CT, because it is three-dimensional, shows you where the object is in the body.?

Along with analyzing causes of death, the virtual exams help to assess the accuracy of medical care in the field. Through imaging, examiners can detect whether medical devices, such as breathing tubes and long needles that can decompress a collapsed lung, performed as expected or fell short. Those analyses spur improvements such as lengthening needles to make sure they penetrate soldiers' sturdy musculature, as well as redesigning body armor to protect against the shrapnel scattered in unpredictable patterns by improvised explosive devices.

Five Actions that can make your Sales Forecasts a Hit ? Business ...

If you are in business or sales, this time of the year is tough. It is the season for budgets and forecasts. ?You are either looking back and thinking what went wrong or taxing your brains on how to get it right for the next year. Or you could be one of the lucky few who raked in more, much much more than you forecasted. Either ways, this is not a good situation to be in. Predictability is key to successful business outcomes. You cannot decide how much to spend and where to spend unless you know with great accuracy how much and where your revenue is going to come from.

What usually happens in the forecasting process is that the senior management asks their sales leaders for a forecast, the sales leaders check with their reps for a ?gut feel? number. The sales reps come up with a number (maybe 10%-20% lower than their gut feel number, to play it safe). The numbers roll up to the senior management. At this stage, the management takes a look at the numbers and checks it with the number they have in their minds (the number that will satisfy the investors/owners), then either discards the sales numbers completely and make up their own numbers or if you are lucky, the numbers get approved. This is quite a hellish process as you can see with lots of stress and heart burn at every stage but no guarantees that the end result is the most accurate version of forecast.

Accurate forecasting is an art ? there is no foolproof method or formula to get it right but there are a few steps or actions that business/sales people can keep in mind that can improve the accuracy of the forecast and make the process easier:

Action #1 ? Understand what Forecasting Is and Is Not ? A forecast is not an aspirational goal, nor a ?quota? or a ?crowd-pleaser? number, nor an administration activity nor a computer program output. It is rather a Projection of ACHIEVABLE sales revenue, based on historical sales data, analysis of market surveys and trends, and salesperson?s estimates. Many people at this stage confuse a sales plan with a sales forecast ? this is disastrous as it affects the entire chain of the business cycle which depends on the achievable number to do its planning to delivery on the forecast.

Action #2 ? Collaborate to Win: To make forecasts more effective, there must be a free-flow of information between all the functions to prevent duplicacy or contradictory data. There has to be respect for different point of views. Each function has its own insights ? delivery timelines, ramp-up plans from engineering, capacity constraints from operations, campaign or events plan from marketing, customer satisfaction scores from customer support? all of these could have an impact on the end revenue results. Hence, it is important to have a strong mechanism in place to efficiently bring together different organizational functions to contribute their inputs in a spirit of collaboration.

Action #3 ? Joining the Dots: A good forecast is never stand-alone. It takes into account trends from the past too like the previous year(s) sales in the same time period to account for the impact of seasonal buying patterns, a similar state of the economy, currency exchange rate fluctuations, availability of resources, marketing campaigns, etc. As Eugene O?Neill famously said ? There is no present or future, only the past, happening over and over again, now. Given that a forecast has to predict the future, it makes good sense to base it on what is known ? the past.

Action #4 ? Reward Accuracy: Like in other management areas, what gets rewarded gets done. Sales people often have the mindset that their job is to sell and not forecast. This mindset is disastrous for the company bottom-line. Accurate forecasts have a huge impact on the company margins so why not put some of it back to reward the source of accuracy. It is important to put in place policies and practices particularly in the job performance evaluation criteria to reinforce the fact that forecasting is important for business success.

Action #5 Track and Improve: At best, a sales forecast is an educated guesstimate built on the basis of certain assumptions. And the basis for assumptions as well as the assumptions themselves change rapidly in the business world. Hence it is critical to review the forecast on a regular basis (fortnightly at the very least) to check if the assumptions still hold good. Measure your forecast accuracy, develop mechanisms and metrics to identify and eliminate the sources of error and plough the learning back into your sales forecasting process. This will ultimately help build confidence in the forecasting process and improve the accuracy so that the entire organization can benefit from better planning.

Meditation as a practice is considered to have begun over 6000 years ago, and is a practice that takes many varieties of form in the present. Some meditation techniques involve focussing on the breath, such as Vipassna , while other practices advocate chanting of a mantra such as Om to focus the mind and keep it in the present.

These meditation practices are passive and take a lot of practice, although the benefits of mastering these specific types of meditation are long lasting and profound. These types of meditation are usually considered more spiritual than other types, although you can certainly practice these methods without a spiritual inclination.

Other types of meditation are more activity based ? my sister meditates every day while washing dishes at the kitchen sink, as she allows her mind to de-focus and relax and finds this very soothing. Other benefits of meditation include a clearer mind, becoming more aligned with one?s life purpose, improved presence (or being in the Now) and increased feelings of wellbeing.

Lack of sleep tied to teen sports injuries

ScienceDaily (Oct. 21, 2012) ? Adolescent athletes who slept eight or more hours each night were 68 percent less likely to be injured than athletes who regularly slept less, according to an abstract presented Oct. 21, at the American Academy of Pediatrics (AAP) National Conference and Exhibition in New Orleans.

For the abstract, "Lack of Sleep is Associated with Increased Risk of Injury in Adolescent Athletes," researchers asked middle and high school athletes (grades 7 to 12) enrolled at the Harvard-Westlake School in Studio City, Calif., to answer questions about the number of sports they played and the time they committed to athletics (at school and through other programs), whether they used a private coach, whether they participated in strength training, how much sleep they got on average each night, and how much they subjectively enjoyed their athletic participation. Seventy percent of the student athletes (112 out of 160 students; 54 males and 58 females; mean age 15) completed the survey, conducted in conjunction with Children's Hospital Los Angeles. Researchers then reviewed those students' school records pertaining to reported athletic injuries.

Hours of sleep per night was significantly associated with a decreased likelihood of injury, according to the study results. In addition, the higher the grade level of the athlete, the greater the likelihood of injury -- 2.3 times greater for each additional grade in school. Gender, weeks of participating in sports per year, hours of participation per week, number of sports, strength training, private coaching and subjective assessments of "having fun in sports" were not significantly associated with injury.

"While other studies have shown that lack of sleep can affect cognitive skills and fine motor skills, nobody has really looked at this subject in terms of the adolescent athletic population," said study author Matthew Milewski, MD.

"When we started this study, we thought the amount of sports played, year-round play, and increased specialization in sports would be much more important for injury risk," said Dr. Milewski. Instead, "what we found is that the two most important facts were hours of sleep and grade in school."

The advanced age risk may reflect a cumulative risk for injury after playing three or four years at the high school level, Milewski said, and older athletes are bigger, faster and stronger.

Mailing Lists versus Forums: Community, Convenience & ClosenessThe way we communicate in groups online naturally changes as our technology evolves. We cannot assume that a technology or format that worked in one decade will work as well in the next.

And yet, there?s a reason why some technologies persist, despite alternatives that seem to offer additional benefits. Email is still with us (and seems likely it will always be, in one form or another), largely because of its asynchronous and convenient nature. Unlike so many other newer online technologies and services that seem to demand our real-time attention, email is happy to be exist in the background on our computers and mobile devices, ready to be reviewed in batches when our time permits.

Yet time and time again, organizations seem to face the same common conundrum ? now that we?ve ?outgrown? our mailing list, how do we move our organization to an online forum?

Let?s review the benefits and drawbacks of different ways people communicate online today in groups, and see if we can?t answer why mailing lists remain popular, despite their flaws.

Mailing Lists

Electronic mailing lists ? also referred as by one of the specific and most widely-used brands, ?listservs? ? have been with us long before the web, since 1984. Their longevity can be attributed to the fact that email is still a commonly used, daily tool by most of us. Because mailing lists exist, in user?s eyes, primarily through one?s email box, they are simple, automatic, and require little end-user thought. The conversations just come to you.

The downside to this automation is one of focus and volume. As mailing lists grow in volume ? whether through an increase in subscribers or through existing subscribers simply posting more and more to the list ? some people find it harder and harder to manage the growth.

As mailing lists grow, they also become more diverse. A topic-focused mailing list of 100 people can grow into a morass when it reaches 1,000 subscribers. Why? Because people start branching out in discussing tangential or social topics that are less focused than the initial mailing list?s topic. Some people find this annoying or hard to manage, while others see it as the normal, healthy growth of a social group.

Mailing lists stink in their ability to easily manage and archive a diverse set of topics. While most mailing lists have web archives, the archives are often difficult to access, search, or browse through. They can only be organized via date, threads, or authors ? but not topics! This is largely the fault of stagnant software development for mailing lists, since they are seen as such old and stale technology. Mailing lists seem to work best when their topics are very well-defined and don?t stray too often outside of that specific topic area (e.g., a mailing list about Lady Gaga would be a better limited topical choice versus one entitled, ?Pop Singers?).

Mailing lists are also greatly dependent upon a user?s email software, and the user?s ability to successfully use all of the program?s features. For instance, email programs since the early 1990s have had the capability to filter messages and organize them into specific folders, to make for easier reading and tracking. Yet still many users don?t know they can do this, or how. Therefore, some users can quickly become overwhelmed by a high-traffic mailing list?s volume.

Mailing lists also have few social aspects built in to the software ? aspects that can enhance a group?s psychological closeness and sense of community. There?s no concept of a user profile with a photo or links so that other users can easily see what that person?s all about. There are no friends lists, or ?ignore? lists for people you don?t want to read emails from (although a person could do this via their email program?s filters).

Web Forums

Web forums come in a diverse set of shapes and sizes. Although web forums have been around nearly since the web itself (circa 1994), people still tend to think of online forums as ?new.? The most widely used commercial web forum software was first developed in 2000 ? 12 years ago! ? and is called vBulletin. Now in its fifth generation, it is a mature product that still has a vibrant developer community to help with add-ons and ways to enhance its functionality and change its looks.

Forum-based online communities are usually a collection of forums, organized around specific topics (which may also have even more discrete sub-forums). A pop singing community could have forums for each popular singer today (?Lady Gaga Forum?), and then sub-forums for even more specific topical discussions (?Lady Gaga Concerts,? ?Lady Gaga Clothing,? etc.). Web forums are simple, too. You go to the forum?s website, register for an account, confirm your account, and usually can begin posting right away.

To try and combat the problem of reminding users to re-visit the forum community, most forums also allow members to subscribe to specific forums or topics of interest, so they can receive updates via email. Usually, however, a user can?t reply via email ? they have to log on to the forum and make their reply there.

Forums allow not only for a greater diversity of topics, they also have much richer social sharing aspects. People can share photos, their user profile, and even create friends lists of people they like on the forums. Alternatively, if a user finds they don?t want to read any more of a particular person?s posts, they can also add them to their ?ignore? list.

Such rich features make forums an obvious and easy choice for most.

The Paradox of Mailing Lists vs Forums

So why does anyone still use mailing lists? Despite their older tech, why does Yahoo Groups, for instance, list over 50,000 health-related mailing lists? Why are the most active cancer support groups still based around mailing lists? Why does most software development still happen via mailing lists?

Herein lies the paradox of mailing lists. Despite all of their faults, they are the easy, lazy choice. That is, because they don?t require any effort on the part of the user to access the topical conversations, they are easier for many people to use, especially if a person belongs to multiple online communities (and who doesn?t nowadays?).

Here?s a personal example. I have not only a half-dozen professional organizations I?m involved in, but I also enjoy about an equal number of hobbies and social interests. Add to that another 4 or 5 areas where I need to keep up as a developer, sysadmin, and owner of a small business. That?s about 18 different communities I?m actively involved in. (And that doesn?t even count the usual array of social networks like Facebook and LinkedIn.)

Imagine they all had web forums as their community of choice. That means in order to stay current with them, I would need to visit 18 different websites a day (in addition to the dozens of other sites I already visit for news, entertainment, distraction, information, research, etc.). This problem is getting worse, too, as more and more websites want you to download their specific app for your Android or iPhone.

I could, alternatively, carefully curate what specific topics I?m interested in for each community, and subscribe to them to receive email alerts when those topics are posted to in the forums. This would require some time up-front, and then some time to keep such lists updated and current. I?m also not sure how receiving so many notices would cut down on my email volume, however, of email volume was the problem I was trying to solve.

That is why mailing lists remain popular ? you can subscribe to dozens of different topical lists, and visit them in your mailbox whenever you want, without having to remember to visit each specific website.

From most organization?s perspective, web forums are the obvious choice. But that choice may not always be so obvious to the organization?s members.
